Re-engaging in my community

Returning to the community is an important part of recovering from a stroke Some people living with stroke say they feel anxious and unprepared when leaving the hospital.  They often say they don’t know what to expect or what questions to ask. 

This section can help you on your stroke recovery journey. It will help you to:

  • Know what questions to ask
  • Set goals for yourself
  • Be more involved in your care and recovery
  • Have a better sense of control
  • Reduce your fear and anxiety

What can I do to take control and manage my health and recovery?

Using this website will help you gain knowledge and skills to help you take charge of your health. This is called self-management.

Self-management can help you:

  • Play an active role in your health and recovery
  • Learn about your stroke and how it has impacted you
  • Develop skills to make good choices for healthy living
  • Improve your confidence

An example of self-management is learning to take your blood pressure at home and knowing what to do when it is high. 

This does not mean you have to do things on your own. Your family and/or caregivers and healthcare team, can support you in making the right choices.  It is also important that family members and caregivers know what questions to ask.
